Day care for people with dementia.

CRITERIA

  • · Member must have dementia

    · Member must live in South Lakeland

    · Referral by Social Worker (An informal approach may be made in the first instance or through a health visitor or community psychiatric nurse, but it will be necessary for a social worker to be involved.)

    TRANSPORT

    Unless the family prefers to make their own arrangements a taxi will pick up the member in the morning and return him/her in the afternoon

    ASSESSMENT OF MEMBER

    During the first six weeks of a new member attending our centre, we make an assessment of the needs of that member. This involves getting to know him/her, their likes and dislikes and tailoring our service to suit them, in consultation with their main carers.

  • Hours: 9.00 a.m. -3.30 p.m. Monday, Tuesday, Wednesday and Thursday

    • Referrals: Via your Social Worker.
